Concord Hospital Medical Group

When you're a patient of a Concord Hospital Medical Group (CHMG) specialty or primary care practice you have access to 345+ clinical providers in 35 locations that represent 28+ sub-specialties. CHMG practices feature modern facilities and new technology, and for your convenience, many practices are co-located with other medical services including radiology, rehabilitation, and laboratory services. Also, CHMG offers primary care weekend appointments at an office visit co-pay. 

Patient Portal

My Patient Connect is a secure Web-based portal that allows you to send messages to your CHMG providers' offices, request appointments, and medication refills, and pay medical bills.

Patient Satisfaction

You are a valued member of your treatment team. Our CHMG providers, staff and managers look to the CHMG Patient Advisory Forum, which includes CHMG patients, to learn about patient needs and to review patient comments to monitor what practices are doing well and how they might improve.

Additionally, our practices measure patient satisfaction through NRC Health, an independent provider of satisfaction surveys and other satisfaction measurement tools. The survey's data is used to assess and improve the performance of the practice.

Electronic Health Record

One electronic health record (EHR) system is used throughout CHMG practices to enhance your treatment and safety by enabling your healthcare providers to see detailed information about your medical history.

  • Keeping you safe by succinctly organizing your diagnosis, medications, allergies, immunizations, and medical tests in one central location that's easily accessible to your healthcare providers;
  • EHR also alerts your providers if you are at risk of certain illnesses and presents reminders for a variety of preventive health measures and screenings, which can improve disease prevention and early diagnosis.

Assistance Programs

Financial counseling and assistance are available to you through Patient Financial Services and if you meet eligibility criteria you can receive help applying for free medications through our Prescription Assistance Program.

  • Possibly qualifying for free or discounted medical care if payment of your healthcare expenses could create a financial hardship for you.

Hospitalist Program

When you are hospitalized our CHMG hospitalists team of board-certified physicians, nurse practitioners, and certified physician assistants oversee your care from admission to discharge. They may order tests and medications, consult with specialists and arrange services you may need when you go home. They also keep your family medicine provider informed of your treatment plan. 

For you, this means:
  • Providers who are available immediately to respond to changes in your condition or to evaluate your progress;
  • More frequent provider visits, which can mean shorter hospital stays.
